Patient- and Family-Centered Care

We're listening: Patient- and -family-centered care at Poudre Valley Health System

Poudre Valley Health System embraces patient- and family-centered care, a philosophy that the best care and environment is built on partnerships with patients, their family members or caregivers, and medical professionals.

This partnership involves sharing policies and programs, collaborating services and fostering an environment where patients and families have a voice in the way that their care is delivered.

Four core principles of patient- and family-centered care

  • Dignity and respect

Listening to and honoring patient and family perspectives, choices, knowledge, values, beliefs and cultural backgrounds and incorporating this into the planning and delivery of care.

  • Information sharing

Sharing complete and unbiased information in useful ways so that patients and families receive timely, complete and accurate information to effectively participate in care and decision-making.

  • Participation

Encouraging and supporting patients and families to participate in care and decision making.

  • Collaboration

Using patients, families and healthcare practitioners to collaborate in policy and program development; implementation and evaluation in facility design; and in professional education, as well as in the delivery of care.

Complaints, compliments and your safety

If you have questions, concerns, or compliments about any aspect of your hospital stay or questions about patient safety, please contact the patient representative at 970.624.1371 at Medical Center of the Rockies or 970.495.7380 at Poudre Valley Hospital, or communicate with us via the GetWell Network.

What to expect while you're a patient

  • Staff members will wash their hands before examining you. Regular handwashing helps prevent infections.
  • We will double-check your name and wristband before giving you medicine or performing tests and procedures.
  • If you are at risk for falling, a staff member will help you in the bathroom, where many hospital falls happen.
  • We will check your vital signs around the clock. We promote rest and sleep, but we also need to wake you for check-ups requested by your doctor to assure you're doing well.
