Caring for you and your loved ones
Patients and physicians may use the services of the Northern Colorado Hospitalists, a group of internal medicine physicians who specialize in treating hospitalized patients at Poudre Valley Hospital and Medical Center of the Rockies.
Hospitalist physicians oversee the care of those patients whose regular doctor, ER physician, surgeon or sub-specialist has requested this service. Hospitalists work closely with the patient's primary care physician to provide a solid continuum of care as the patient enters and exits the hospital.
Your physician will receive a summary of your hospitalization, including comprehensive notes, test results, treatments administered, medication changes and discharge instructions. Your physician is welcome to visit you during your stay and may contact the hospitalist caring for you for any updates. The hospitalist program allows your personal physician increased availability for office appointments. It also ensures that each patient receives expert and efficient care while in the hospital.
This ensures good communication between physicians which facilitates better overall care for the patient. Hospitalist doctors can also help coordinate care as a patient transitions from the hospital to a skilled nursing facility or home. They assist with geriatric and end of life care as well.
A hospitalist physician is at PVH and MCR around the clock to care for patients; when an emergency arises, they are onsite and ready to care for their patients. The hospitalist physicians hold family meetings with each patient's family, giving ample opportunity to answer family questions and address concerns. They partner with PVHS staff members and the family to provide patient-and family-centered care.
